Abstract: 

Background & Objective: Patients suffering from Cardiac arrest (CA) have poor prognosis and survival. The association of pre-arrest comorbidity with unsuccessful resuscitation in patients with CA is far from clear. The aim of the present study was to investigate the association between pre-existing comorbidity and unsuccessful resuscitation following CA in Iranian patients. Materials & Methods: Data of all the patients with CA (1320 patients) who were admitted to Baqiyatallah Hospital (Tehran, Iran), and underwent cardiopulmonary resuscitation (CPR) from 2018 to 2020 were retrospectively reviewed. We analyzed the association of comorbidity data with the mortality rate and unsuccessful CPR. Results: In the present study, 794 (60. 2%) patients were female. Most CAs occurred in the hospital (1289). The case fatality rate (CFR) of CA was 69. 92%. Additionally, of the 1320 patients, CPR was unsuccessful in 1271 (96. 3%) patients. Our data analysis revealed that gender had no significant relationship with the mortality rate and unsuccessful CPR. However, age was significantly associated with mortality, but not with unsuccessful CPR. CA, respiratory distress, and impaired consciousness were significantly associated with mortality. Furthermore, cardiomyopathy, coronary artery bypass grafting (CABG), renal disease (P=0. 032), and poisoning had a significant correlation with mortality. In addition, CABG and congestive heart failure (CHF) were significantly correlated with the unsuccessful CPR. Conclusion: The present study suggested that CABG and CHF had a significant relationship with unsuccessful resuscitation. Furthermore, cardiomyopathy, CABG, renal disease, and poisoning were significantly associated with mortality.

Abstract: 

Tracheostomy is a widely used procedure for airway maintenance and long-term ventilation of critically ill patients in ICUs or operating rooms. Although several complications, such as bleeding, leakage, infection, and stenosis, have been previously reported due to tracheostomy, cuff herniation is a rare complication of this commonly performed procedure. The first sign of herniation is a sudden drop in oxygen saturation. Since cuff herniation is not among the first differential diagnoses for a drop in oxygen saturation, it should be considered in the absence of other common causative agents. In this study, we delineate a case of tracheostomy cuff herniation after Cardiac arrest in a 64-year-old woman who underwent coronary artery bypass graft surgery. The patient experienced a cerebrovascular accident and mediastinitis postoperatively, necessitating tracheostomy due to extended tracheal intubation, during which the cuff herniated for the first time. Furthermore, a repeat tracheostomy was performed,however, the patient ultimately expired due to complications from mediastinitis.

Abstract: 

Introduction: The survival rates and marker of survival in children with Cardiac arrest after congenital heart surgery are unknown.Methods: In this one-year study, we identified children less than 7 years old in our pediatric Cardiac intensive care unit database who had a postoperative Cardiac arrest. Parameters from perioperative, pre-arrest and resuscitation periods were analyzed for these patients. Comparisons were made between survivors and non-survivors.Results: Of 529 children under seven years old who underwent corrective heart surgery, 59 (11%) sustained a documented Cardiac arrest; of these 22 (37%) survived and their vital signs returned. Perioperative parameters (age, weight, pump time), ventricular physiology, O2 saturation, and HCO3 did not influence outcome. Greater use of isotropic agents was not associated with higher mortality. A significant relationship was seen between history of Cardiac arrest and success of resuscitation (P<0.001).Discussion: Success rate of resuscitation in cases of hemodynamic compromise (i.e. low mean arterial blood pressure) is low. Patients with single ventricular physiology or history of Cardiac arrest do not stand a higher risk of death following arrest.
